Background methods?

So I got to the "Backgrounds" section in the Player's Handbook, and I was a little confused on how this is exactly done. I'm currently under the impression theres only 2 ways to make a background in 5e: 1. You can choose a sample background, and flesh it out a bit more from the suggestions, like in the case of a "Charlatan" you specify more about your manipulative ways, where you grew up, what made you into such a trickster etc. (and I'm wondering as well for the sample backgrounds, the tables you roll for such as the Charlatan's "favorite schemes" or the Criminal's "Criminal Specialty" do you include what your favorite scheme or speciality is in your background, or somewhere else?). 2. You tweak some of the features of the sample backgrounds to better suit your character or the campaign setting. Is there a 3rd way to make a background completely from scratch? Perhaps by following the same rules on customizing a background, by choosing one feature, two skills, and a total of two tool proficiencys or langauges (I believe that means I can pick one tool proficiency and one langauge instead of having to pick two tool proficiencys or two languages). And finally, I know the main setting for 5e is the "Multiverse" but what planet exactly is it supposed to be on? Perhaps it's just any planet you want it to be, but I'm very new to this game haha. Thanks!

I think the options you list as #2 & #3 are one and the same.

In either case, your #3 is what I see as the intent. You choose different components from the sample backgrounds, to assemble your own. EDIT: At this point, anything more would have to be done by the DM; or, at the very least, approved by the DM.

NOTE: I think the intent of the tool/language option is to take a total of 2; they can be 2 tools, 2 languages, or 1 of each.
