Do I understand the rule correctly:

  1. Everyone insight (usually 30 ft) of the creature has to make the save every turn except if they avert their eyes (disadvantage to attack the creature). This take no action from the creature whatsoever.

Correct. Simply looking at such a creature is hazardous to your health. No action is required on its part.
