Great Weapon Fighting

So I have a question about great weapon fighting, specifically how its wording applies to additional types of damage.  It states "When you roll a 1 or a 2 on a damage die for an attack you make with a melee weapon that you are wielding with two hands , you can reroll the die and must use the new roll".  I have a few questions

1:  Does this apply more than once to the same weapon:  Say I have a greatsword or maul and roll snakeyes, do I reroll one or both die?

2:  Does this apply to any bonus die that may have been gained through class abilities or spells?  My gut tells me no but the wording leaves it kind of up in the air (and I'm unsure of the intent or not).  This is especially fuzzy when things add to damage rather than add an extra damge type. An example; say a paladin of vengance smacks someone indicated by his hunter's mark.  Said paladin is also 11th level so he adds an extra 1d8 radiant damage onto his every attack.  Finally he decides to smite the fellow as well using a 1st level spell.  He's now rolling 2d6 for his greatsword, another 1d6 extra from his hunters mark, as well as 3d8 radiant damage total from his divine smite and improved divine smite.  Say some wild improbablity happens and he rolls 1s and 2s on all of those dice; can he re-roll his origional 2d6 only?  The modified 3d6 (the wording is that it deals "extra damage" with no typing indicated)?  Or is he able to re-roll every die used in the attack?

Again my gut tells me its just the origional 2d6 and maybe the extra d6 from hunter's mark, but I'm not sure.

3:  What about crits?  Let's leave the previous ultra complicated example out of this and say said paladin is only first level and crits, launching 4d6 at his foes.  Can the crit dice be re-rolled if they come up poor?

1. Yes

2. Unsure, but I'd say it only applies to the damage dice from the weapon itself. Otherwise, it would be ridiculously overpowered. Could be a DM call.

3. Yes, you can also reroll the double dice from the crits.

Intent is hard to judge, but the RAW is fairly clear: it states a damage die for an attack you make. Not a weapon damage die for example.

There are no limitations on how many dice you get to reroll, just that it has to have rolled a 1 or 2.

Paladin smite, or warlock hex or hunter's mark, or crit extra dice are all damage dice from an attack you make, and thus would seem to be rerolled if they got a 1 or 2.

You should be able to re-roll the 1s and 2s from all of the above: both dice of a greatsword or maul, critical hit dice, and additional dice from spells or features such as Sneak Attack or smite spells.

Note that even if you are potentially re-rolling all the dice from a sneak attack, smite, divine strike, superiority die, etc., the actual damage bonus is less than or equal to +1 per die being rolled.  Without bonus dice, a greatsword itself gets 1.33 bonus damage, which is less than the +2 granted by the Duelist style.  Of course, a greatsword does more damage than any one-handed weapon to begin with, but i think it's a pretty fair trade for the lost shield's +2 AC.
