What are the specifics required to initiate and maintain stealth/hidden?

Searching the rules forums turned up nothing current. And my rules questions arises from the "Surviving level 1" thread in Player Help.

It seems that PC/npcs cannot hide in combat and be able to attack any target at the same time: If I can see you, can can see me, erego I cannot be hidden from you, ergo You cannot be hidden and attack a target. It would seem only halflings and wood elf can hide in combat. That seems silly to basically restrict rogue to those two races. It would seem getting a bonus action to hide is basically a wasted feature.

Rogue doesn't need to stealth.  If one of your allies is within 5 feet of the enemy, you get sneak attack.  Flanking no longer exists and there is no range requirement for sneak attack.  You can stand in the middle of a brightly lit road and shoot 80 feet away and land a sneak attack so long as your Fighter friend is up in their business.  If you want to get sneak attacks off at the start of combat, the Assasin archetype gets advantage on anyone who hasn't acted in combat yet.

I fail to see the need for you to use stealth in combat.  Why do you need to do this?
