Have a question, can you make a grapple check for each attack you have in one attack action or is it only one grapple per attack action?

1 grapple per attack.

So at level 5, you can make 2 grapple attempts.

And a fighter can make 4 attempts at level 11 (8 with an action surge, 9 with haste).

Note that you still need to perform an attack action, so it doesn't work with TWF or OAs (though i allow it for any attack).

Keep in mind that creatures like the roper have a multiple tendril attack that automatically grapples on a successful hit.  Things can be a bit different for monsters than for PCs.

For PCs the grapple action is a special attack action.  The rules state clearly that as long as the target creature is up to one size larger than you, and you have a free hand, you can use one of your attack actions to attempt to grapple it.  In theory, yes, you could use your second free hand to grapple a second creature as long as you have another attack action.

It doesn't appear that an off-hand attack is allowed to be used as a grapple, as off-hand attacks use up a bonus action and grappling requires the use of an attack action.
