Does the spell end if they make the first saving throw? Seems underpowered for a level four spell?!

Considering it says "On a successful save, the spell ends", I'd have to say yes, if they make the saving throw, the spell ends. Particularly since it doesn't say anything about multiple saves, so there is only one saving throw. 

 

As for being underpowered, Compulsion, Confusion,  and Dominate Beast are all 4th level spells avoided entirely by a save, so no, I'd say it's on par with those.

Does seem a bit lame that the target has to fail two saves to have the spell do anything at all.

And here, I was thinking the spell sounded silly because it's titled Phantasmal Killer and it doesn't kill anything; or, do consecutive failed saves cause death? I don't have access to my PHB at the moment.

I had a PF witch that killed a few baddies with it......haven't seen it used in 5th yet.........one of those spells that isn't great for players but can be horrid when used on them

IMO:  It's supposed to be...

 

"On a failed save, the target becomes frightened for for 1 minute".  

 

Rather then until the spell ends.
