Questions from real games

I apologize if any of these are repeat questions. Most of the following situations came up in-game and we were unsure how to handle them. Some of these are more theoretical. The list is long; thank you in advance for your help!

1) A gnome and an elf are both listening at a door when the door is opened from the other side, a surprise attack is made, and combat initiates. For the initial surprise attack, where do the gnome and elf stand? (The players made it clear the elf stood over the gnome while listening through the door.)

2) Presumably, after the initial attack, one of the two creatures would side-step to the next available space. Is this correct, or must they wait until their turn to do so?

3) In-game, we forced the higher-initiative gnome to step aside. The surprise attack was made against the elf, who was immediately knocked unconscious. The bugbear proceeded to step into the space containing the fallen elf's unconscious body. On the gnome's turn, she wishes to heal the elf with a touch skill. Does this impose an attack of opportunity from the bugbear standing atop the fallen elf?

4) Once healed, the elf is now conscious but prone, with a bugbear atop him. Does the bugbear realize the elf is awake? Does this change if the elf was healed via Word of Healing (which is not a touch spell)?

5) On the elf's turn, he wishes to stand up. Does this provoke an attack of opportunity from the bugbear?

6) After standing, there is no open space adjacent to stay in. The elf wishes to attack the bugbear. Is he permitted to do so?

7) Realizing he must end his turn in an empty space, the elf retreats to an open location. This will provoke an opportunity strike. Because the elf spent half his movement standing, it is feasible there is no open space within the remainder of his move- what if this occurs, and the elf cannot reach an open space? (Does this force the elf to take the Dash action?)

8) In another occurance, a character is knocked prone. An enemy stands next to them. The prone character stands up. Does this provoke opportunity?

9) A character is dangling from a ledge during battle (having made a dexterity throw to catch the edge of a pit trap). How much of their movement is required to pull themself up? Is a strength check required, or a minimum strength level necessary? With a high strength, can the character pull themself up into a crouching position, or must they pull themself into a prone position?

10) Loading mechanic on an item, like a crossbow, seems confusing to me. We interpreted it as requiring one action to load, and another action to fire. Upon review, it seems instead that Loading prevents any extra attacks from being used. To clarify then, a creature may load and fire a crossbow during the same action? But if the creature could normally attack more than once, that character only gets one attack with a crossbow? With the crossbow expert feat, would this character get as many attacks as normal (such as a fighter firing 4 crossbow bolts in one round)?

11) The transmutation cantrp Shillelagh changes a quarterstaff's weapon die to d8. This weapon is normally versatile, and can have a d8. Does that mean with Shillelagh active, the versatile aspect becomes d10?

12) The Great Weapon Master says if you score a critical or bring something down to 0, you get a bonus action attack. Normally, a creature can only have one bonus action per turn. If on the bonus action, this feat would trigger again, does this mean the character may not use the second triggered bonus action? (i.e. can this feat chain mutliple times?) Can it activate on the bonus attack of another ability, such as dual wielding?

13) How many spells does a lvl 1 wizard with 16 Intelligence "know"? Not how many can he prepare, but how many can he have in his spellbook? When he levels up, can he replace spells in his spellbook? Can he add a new one? When he gains lvl 2 spell slots, can he just pick anything from the wizard spell list and add it to his spellbook for free?

14) When does the 50gp cost and associated time come into play for a wizard adding a spell to his book? Can a wizard transcribe a spell from a scroll (like a scroll of augury) to his spellbook? Can a wizard use any spell so added to his book, or must it be one in the wizard list?

15) Do sorcerers and warlocks have to complete any kind of rest after leveling up before using new spells, or are they automatically available for use, possibly in the same battle which they leveled up in?

16) A lvl 11 ranger has the Whirlwind Attack feature. Assuming the ranger is surrounded, what is the maximum number of attacks he can make with just his action alone? Does dual-wielding permit him to make any additional attacks? Can the feat Great Weapon Master trigger an provide an extra attack? If the ranger is using a weapon with Reach, can the Horde Breaker feature allow the ranger an additional attack?

17) A lvl 11 ranger has the Volley feature. Assuming one monster per square and optimal selection of the attack, what is the highest number of attacks the ranger may make with this one action? (ten feet radius is two squars left, two squars right, two squares up, two squares down. I count 16 attacks)

18) Assuming a lvl 5 ranger with the Horde Breaker feature and dual wielding, how many attacks can be made? (1 normal + 1 extra + 1 bonus from dual wielding + 1 horde breaker = 4; is this correct?)

19) Can any dragon perform a fly-by attack using its breath weapon? i.e. Fly low over a group and breath fire on them?

20) A warlock has The Fiend pact, granting him the Dark One's Blessing feature. If a creature would die from two effects simultaneously (such as acid from the warlock and fire from a sorcerer which trigger on the creature's turn), who "gets" the kill?

21) The warlock later chooses the Hurl Through Hell feature, which says it may be actived whenever "you hit a creature with an attack." I read this to mean it doesn't matter what kind of attack, whether melee, range, or spell. Is this correct?

22) What happens to a warlock if their patron dies? Can a warlock attack his patron? Does he lose his power if the patron dies? To continue gaining warlock levels, must they forge a new pact?

23) Could a tiefling be considered a warlock merely because of his blood, or must the tiefling make a formal pact?

I think that you should stop calling it disengaging since there's a game mechanic called disengage which stops OA's from doing exactly what you're talking about.  It's confusing.

No, only moving out of the reach of another creature will provoke OA's caused by movement unless otherwise specified.  Some spells generate OA's for example.  You can run around the target, as long as you don't leave its reach nothing happens.  Keep in mind also that creatures can have various reach sizes.  And of course, there's ways to avoid those OA's, namely by using the Disengage action.

Great answers, thank you! (EDIT: for any wondering, I deleted a post, as the question I asked was answered by a post made whilest I typed my post. That deleted post has now been quoted. Sorry for any confusion.)

I think that's pretty much all fluff.  Except for the mechanical source itself (asmodeus, the old one, fairy magic, etc..) but even then you could refluff.

One thing of note: Moving into/within a square occupied by another person (enemy or ally) is considered difficult terrain, so 1 ft of movement costs and additional 1 ft of movement (in addition to other forms of difficult terrain stacking on that, usually). By RAW you can't walk into/through an enemy's space unless it's 2 sizes bigger or smaller (halflings have a specific rule that trumps this).

Obviously this is a bit situational since the Elf's body was sorta like an object at that point as he was on the ground unconscious and woke up with a bugbear butt over his face, so the Elf can crawl out of the way to try and stand that turn or next (depending on what open spaces are available around him), and cannot end his turn in another creautre's spot. So he'll essentially have to disengage+crawl or dash+crawl if he can, or he's basically stuck there.
