I apologize if there a thread for this already but I could not get the Search tool to work for some reason.

When I think of raging, I think of you are using more of a simple reasoning, not if I place this greataxe 3 inches below his left thigh I will sever a major artery and he will bleed out.

Am I really overthinking this too much?

Or is there really a way to justify a Barbarian "while Raging" has the concentration to land strikes in more vital areas rather than just be thinking i swing hard enough to cut him in half he will stop moving.

The relevant rules are:

Rage, Players Handbook, page 48

Sneak Attack, Basic Rules, page 27

My ruling is:

Yes, the barbarian may apply sneak attack damage while raging if he or she meets the requirements for sneak attack (has advantage or another enemy of the target is within 5 feet). The only things a barbarian is prohibited from concentrating on are spells.

But you or your DM may make a different ruling based on this or other criteria.

One quick note, the weapon needs to be ranged or finesse. A greataxe is neither.

Rapier and shortsword are the only two weapons that I see working, and I could see a barbarian duel wielding either or......thats a nice varient build actually.....Though it is a bit MAD since I would like High STR and DEX, and decent CON

Sneak attack isn't about precision -- precision is part of the attack roll abstraction.  A rogue with sneak attack isn't any more precise than a fighter without sneak attack.  It is rather that the attack exploits something about the situation -- e.g., the enemy is engaged with one guy, and so you stab the enemy in the back while s/he isn't watching.  Or you spit in the enemy's face and then hit him while he can't see.  Think 'fighting dirty,' not 'striking with precision.'

If you are concerned about verisimlitude, require the player to narrate what he's doing to take advantage of the sneak attack opportunity while raging.  That creates a vivid image for everyone, and you'll find that players can be very creative when it comes to thinking of how their characters do awesome things.
