Has Jeremy made an official ruling on whether sneak attack can be used a second time when you attack with a reaction during another creature's turn?

Yes, and it's pretty consistent: "turn" means whoever's acting, "round" means from the start of your turn to the start of your next. Opportunity attacks and other reactions are a chance for a second sneak attack.

Yep, once per turn means that you can do it once on every turn of the initiative order if you can somehow gain an attack.  Battle Master Fighters can grant attacks to allies, OA's, readied actions, etc...

I tweeted a long time ago and asked him this question. Is there actually somwhere Crawford answered the question?

 

I asked online in various forums months ago and the response was about 50-4 that you could. And two of the four 'nos' changed their mind. One of the remaining four never responded further in that particular thread. And the fourth had a nonstandard definition of turn.

Sageadvice.eu archives many Crawford and Mearls clarifications, easily searchable. In this case, http://www.sageadvice.eu/2014/09/12/sneak-turn/ discusses this. Mearls started out thinking it was 1/round because he couldn't reach his PHB (cat gravity is fickle), but once he was reminded it was 1/turn, he points out it worked on off-turn attacks.

Even being applicable every turn isn't a problem; there are limited ways to gain off-turn attacks. So, it isn't like a rogue will ever be able to "spam" the ability on every turn (well, at least not unless there are very few combatants/turns).
