Statement of Acceptance Q - I'm a DBA

Jan 27, 2009 16:22:36 My company is a DBA/Sole Proprieter, which essentially means I am an individual. However, the Statement of Acceptance only asks for name on the Individual option, and the Company option asks for incorporation jurisdiction, which technically does not apply to my company.

So, which option should a DBA check?

I'm assuming individual, and I will add the DBA info to that line.

Please confirm.
