The only types of powers which provoke (unless they for some reason state otherwise in the text) are ones keyed as "Ranged" or "Area." That means "Close" (burst or blast) and "Melee" flagged powers do not provoke.

The actual number of squares a power extends, or the power source (arcane, martial, divine etc.) is irrelevant to whether it provokes or not.

Not precisely. There are a number of "Melee touch" powers, such as the "cure * wounds" spells. But you are right that whether it provokes or not is based on the melee part (meaning they don't provoke) and not the touch part. The touch keyword just means you have to touch with your natural reach, without using a reach weapon.
