Dueling rules/stories in D & D

In my D & D or the group I play with, we have a rule that no matter what the module, world, level of character, etc, if a player has a duel to the death in the and they lose, they are dead forever. No wish, device, spell, etc will save them. We decided that if you are going to have a duel to the death then the threat has to be real and it gives it a realistic dramatic feeling. There is nothing scarier seeing a player in a duel with a character that they have been playing for years putting it all on the line. Any good stories or rules people have concerning dueling?

I was a DM in a game where pirates challenged the PCs to duel, champion versus champion in order to resolve their deadlock. Both sides buffed their champions with spells and potions. The pirates used disguise to make their fighter heavy hitter look like their wizard in order to trick the PC champion into the incorrect tactics. When, after a few rounds, this deception was uncovered (and the pirate had the slight edge), the rest of the PCs decided that was playing dirty, and they all attacked.
(The real wizard was waiting invisibly in case the heroes dishonoured the agreement.)

Or gm allowd something he called "challenge by the light" this often involved summoning a lawful outsider to be the impartial judge. Whoever lost the duel (brought to 0 HP) was taken by the outsider to their native plane, one posession of the looser was given to the winner by their choosing, the remainging items were given to the loosers group to compensate them for their loss of the character.

IN a Warcraft campaign, I was playing a duelist who focused on the rapier. He was a cocky son of a b, and decided to taunt and degrade an orc into a duel. He was killed by said orc, and my DM decided he was going to use the "coming back wrong" chart from the Heroes of Horror book...

He rolled the one that makes the character shun all earthly possessions. Which was fantastic for a sword-focused character...
